**DOCUMENTARY /TELEVISION/FILM SUPPORT**

We are the primary contact for processing documentary/television/film production support requests. In order to initiate the process, please click on the link below to open the form. Return completed document to this office via e-mail attachment.  We will advise you via e-mail upon receipt of your request.  At present, we can only provide the questionaire in PDF format.   Your request must be submitted 30 days in advance of your shooting date. We will review and provide you with a decision. Pertinent information listed below.
